 When Purchasing PTA Memberships, please remember to make sure the QTY section reflects the number of people you are signing up for membership.

**NOTE- If only purchasing one PTA membership please fill out necessary information for PTA Member 1 and answer the Volunteer Trained question.  List "None" for PTA Member 2 and select "Not Applicable" for the Volunteer Trained question. See example to the right.
